Applicant must submit their current, signed passport. Visas are stamped into a passport, therefore the original passport is required.
Applicant must submit two (2) identical, color, passport style photographs. The photos must be recent and generally should not be the same used for the passport itself. Please write first and last names of applicant in pencil on back of photos
Applicant must submit a computer-generated itinerary from a travel agency or online booking tool (such as an e-confirmation or online booking) showing departure from the US and arrival/departure for the destination.
Note, if traveling via private charter the charters manifest should suffice.
Applicant must submit a copy of a confirmed hotel reservation.
Applicant must provide their three most recent bank statements. Personal details may be obscured, such as account numbers, or specific purchases, etc. However, the critical information that must remain visible is the account holder's name and final balance of each statement.
Although applicant may be submitting documents along with other family members, when submitted to an embassy or consulate, applications are treated individually. If applicant is under 18 years old, please view the following link.
Applicant must complete the visa application using the fillable PDF via their web browser - hand written applications will not be accepted. Print and sign TWO (2) completed copies of the application. Applications should be printed single-sided and signed in blue or black ink only.
Arrival dates provided on the application, and the travel itinerary must exactly match. The visa should be valid based on the dates provided in the application and you cannot enter before that date (please check your visa for valid entry date).
Approval may be required for certain applicants, and may extend processing time for several weeks.
