Applicant must submit their current, signed passport. Visas are stamped into a passport, therefore the original passport is required.
Applicant must submit two (2) identical, color, passport style photographs. The photos must be recent and generally should not be the same used for the passport itself.
Applicant must submit a computer-generated itinerary from a travel agency or online booking tool (such as an e-confirmation or online booking) showing departure from the US and arrival/departure for the destination.
Note, if traveling via private charter the charters manifest should suffice.
Applicant must submit a tourist voucher obtained from a Travel Agency or Hotel in Belarus. The voucher must include the dates and length of stay, the applicants name as printed in their passport and specify the number of entries they are requesting for the visa.

Applicant must submit a completed visa application. Original signature required. Applications should be printed single-sided and signed in blue or black ink only. You must not use any white-out on your visa application. If you make an error while completing the form, please discard and complete a new form. The Embassy will reject any application forms with white-out.
Applicants born in Belarus MUST submit a Certificate of Renunciation of Citizenship from the Belarusian Embassy.
Effective October 1, 2000 medical insurance is required for all foreign citizens traveling to the Republic of Belarus. US citizens may purchase the insurance upon arrival at Minsk National Airport.
